Step 1: Complete the INTENT FORM

Step 2: Complete the REGISTRATION FORMS
If you do not have access to a printer email or call 801-622-3580 to request a copy be mailed to you.

- Verification of Address Form --> Plan Ahead: This document requires two forms of proof of residence OR a notarized Residence Disclosure
- Kindergarten Dental Exam Form --> Plan Ahead: This document requires a dentist's signature.
- Kindergarten Medical Examination Form --> Plan Ahead: This document requires a doctor's signature.

Step 4: BRING original documents to Weber Online
An original birth certificate is required for Kindergarten registration. The Weber Online enrollment specialist will set up a time for you to bring this and any other needed documentation with you to Weber Online headquarters.
